Complete and revisit surveys

Lotti includes three fixed questionnaires: CFQ-11, GHQ-12, and PANAS. Add the survey you use to a dashboard, then select the + action on its chart whenever you want to record another response. The dashboard is both the launch point and the place where completed scores become a trend.

The examples below launch the real PANAS task from the Colony operations dashboard. The questionnaire and its navigation come from the same production survey runner used by the app.

Add a survey to a dashboard​

Open Settings → Definitions → Dashboards, edit a dashboard, and add a chart from Surveys. The available instruments are:

  • CFQ-11, the Chalder Fatigue Scale;
  • GHQ-12, the General Health Questionnaire; and
  • PANAS, the Positive and Negative Affect Schedule.

Return to Dashboards and open the dashboard. The chart shows submitted results for its instrument. Select its + action to begin a new response. Each chart launches its matching questionnaire with a fixed set of questions; there is no generic form that adapts its questions to your recorded data.

Answer the current question​

The instruction step explains the requested time frame and the meaning of the answer scale. Select NEXT to start. PANAS then presents 20 affect words one at a time and asks for one choice across the five-point scale. The instrument's instructions, affect words, completion copy, and scale labels follow the app language; this English screenshot shows Very slightly or not at all through Extremely.

The other bundled surveys use their own fixed question and scoring definitions. Choose the response that matches the wording and time frame shown by the instrument; Lotti does not reinterpret or auto-fill answers.

Submit or cancel deliberately​

Continue through the completion step to submit. Lotti stores the raw task result and its calculated score buckets as a survey entry in the Journal. The dashboard reads those entries back into its chart, so another completed survey adds another dated observation.

Closing the task asks whether to discard the current result. A cancelled survey is not stored, even if it was partially answered. If the response matters, finish the completion step before leaving the sheet.

Understand the saved scores​

The persisted entry keeps both the individual answers and the calculated summary used by charts:

  • CFQ-11 produces its configured fatigue score;
  • GHQ-12 produces its configured general-health score; and
  • PANAS produces separate Positive Affect and Negative Affect totals.

Open the corresponding Journal entry to inspect the saved survey summary. Use the dashboard for change over time and the Journal for the dated source record. Survey charts do not replace the questionnaire's instructions, and Lotti does not provide a survey authoring system for changing these bundled definitions.
